- Immigration Pass Application Form
- Copy of Principal’s Passport (Biodata Page)
- Copy of Dependant’s Passport (Biodata Page)
- Proof of family relationship (Must be translated and certified by the Malaysian Embassy/High Commission/Consulate or by the Malaysian Ministry of Foreign Affairs)
